Individual Health Record
The Individual Health Record is a summary of your GP record, which will let GPs and nurses working in your local Out of Hours Service see some basic information about your medical history.
They will only be able to access this information if you need care from the Out of Hours Service and you tell them that they can see it.
Medical staff working in your local out of hours service need as much information about you as possible in order to provide the best possible care. The Individual Health Record allows them to see some of the key information kept about your care by your GP
Without this essential information the medical staff providing care when your GP surgery is closed at evenings and weekends, only know what you tell them. Yet, when you are unwell its sometimes easy to forget details of any medication or allergies.
The details included on your Individual Health Record are:
• Your name, address and contact details
• Details of your current GP practice
• What medication you are or have been taking
• Medical problems you’ve seen your GP about
• Allergies
• Recent tests results, for example, blood tests and x-rays
Only the last two years medication history and the previous year’s test results will be available.
The only people who will be able to see your Individual Health Record are the GPs and nurses working in your local Out of Hours Service. They can only access your information if they are treating you when your GP surgery is closed, and you give your permission.
If there is any reason you cannot give permission, for example in an emergency, then the medical staff will always act in your best interests. If they decide to access your record, they will have to record why.
Patient information from this practice is now being made available for medical staff working in the out of hours services to view. This information is known as the Individual Health Record. If you would like to opt-out, please contact the surgery.
