    Summary

    The Cardiff Community Resource Team (CRT), work with people in their own home to maximise functional independence in activities of daily living (ADL).  This reduces the need for admission into hospital and longer-term Social Services care services.  The service also offers therapeutic intervention and reablement support to people following an admission into hospital, so that the return home can be as timely as possible.  They provide patient-centred therapeutic intervention and support.  They work in partnership with the individual towards achieving goals that have been jointly identified.  The team includes Occupational Therapists, Social Workers, Physiotherapists, Speech and Language Therapists, Dieticians, and Nurses. They also work collaboratively, with the Local Authority to provide the care element of the service. 

    How to access the service:

    1. Referral by Health Professionals

    A referral to the CRT can be submitted electronically by Health Professionals within the community, care home staff and hospital staff.
