Summary
The Rhondda and Taff Ely Older Persons Day Services’ provide support to individuals living with a diagnosis of dementia living in the community, regardless of age. The aim of the service is to enhance social and mental well-being through positive interaction and meaningful activity. The individual will have access to a multidisciplinary team, therefore reducing the impact of dementia on the person and their families.
Can anyone use this service?
Patients who attend day service will be required to meet the criteria set for the service to ensure the individuals needs can be met.
A referral will need to be completed and sent directly to the day unit management team, the referral will be screened by the managers prior to an assessment being offered.
What to expect
There is a strong emphasis on maintaining function and ability to carry out everyday tasks. Individuals will be able to engage in occupational activities, whilst enhancing social engagement in a friendly, homely environment. The day unit has Registered Mental Health Nurses who monitor individual’s mental health as part of the multidisciplinary team.
Referral Criteria: Person must experience memory problems. An open referral so anyone can refer in. Assessments will be completed at unit and can be discussed further with the service.
