    1. Referral via GP

    You must have a blood form from your GP to access the blood test service.

    Blood test services for adults within the Betsi Cadwaladr University Health Board area are based at Buckley Medical Centre, Colwyn Bay Community Hospital, Connah's Quay Health Centre, Deeside Community Hospital, Denbigh Hospital, Flint Health and Wellbeing Centre, Grove Road Health Centre, Holywell Community Hospital, Hope Community Health Centre, Mold Community Hospital, Royal Alexandra Hospital, Ruthin Hospital (over 18's only) and Wrexham Maelor Hospital.

    You can book a blood test online via the online booking system.  Please speak to your GP for areas not covered by the online booking system.

    If you can’t book an appointment online, you can contact the blood test service on telephone number 03000 850047, between 8.00am and 5.00pm,Monday to Friday.  

    For Denbigh and Ruthin clinics, you can contact the blood test service on telephone number 03000 850025 Option 4, between 10.00am and 2.00pm, Monday to Friday.

    Note the service is intended for patients registered in Wales.

    Be advised that if your request is from a GP based in England, your results will not get back to your GP.  Contact your GP for advice.

    If your tests are in relation to private healthcare, ensure this is indicated on your booking. You will be invoiced for the cost of sample collection and tests. Contact your private healthcare clinician for advice.

    Children must have a blood form from your GP to access the blood test service.

    Blood test services for children, aged under 10, within the Betsi Cadwaladr University Health Board area are based at Wrexham Maelor Hospital and Glan Clwyd Hospital.  To book an appointment, please contact Wrexham Maelor Hospital on telephone number 0300 848012 or Glan Clwyd Hospital on telephone number 01745 534871.  Please speak to your GP for areas not covered by Wrexham Maelor Hospital and Glan Clwyd Hospital.

    You can book a blood test online via the online booking system.  Please speak to your GP / midwife for areas not covered by the online booking system.
