Summary
The Occupational Therapy Service throughout Gwent provides assessment and treatment for a range of client groups. Treatment is selected through specific occupations with the purpose of enabling individuals to reach their maximum level of function and independence in all aspects of life. Occupational therapists work within multidisciplinary and multi-agency teams in both acute and community settings. The range of specialities offered is vast and includes Adult Physical Services, Adults with Mental Health Difficulties and Adults with Learning Disabilities.
How to access the service:
1. Referral by GP / Health Professional
You can be referred to the service by your GP / Health Professional.
Your GP / Health Professional can complete a referral form.
Completed forms should be returned by post to: Occupational Therapy, Therapy Services Booking Centre, ABUHB Headquarters, St Cadocs Hospital, Lodge Road, Caerleon, NP18 3XQ or emailed to CentralBookingTeam.abb@wales.nhs.uk. In the subject line you must enter: Request form OT Adult. You may also fax the completed form to 01633 431720.
