Summary
Occupational Therapists recognise that being able to perform daily activities is crucial to health and wellbeing. Occupational Therapists (OT's):
- Work with people of all ages and backgrounds who are affected by accident, physical and mental illness, disability or ageing
- Provide help and training in daily activities such as bathing, dressing, eating, gardening, working and learning
- Offer advice on adapting the home, workplace or school to meet individual needs or recommend equipment to assist with activities of daily living.
How to access the service:
1. Referral by GP / Health Professional / Other Professional
You can be referred to the service by Paediatricians, GPs, school nurses and other health professionals. You may also be referred to the service by Social Services and children's teams, Educational psychologists and additional learning needs coordinators (ALNCOs). The referral should be filled out in full by a referrer who has first-hand knowledge of the child’s difficulties.

Referral for for children living in Conwy or Denbighshire
Usually the first assessment is carried out over the telephone so please ensure all the numbers are correct and that you have stated any communication or language needs. Please also include an email address. Requests may be returned if more information is needed.
Completed forms can either be returned by post to Children's Occupational Therapy, Conwy and Denbighshire, Royal Alexandra Hospital, Marine Drive, Rhyl, LL18 3AS or via email to: BCU.ChildrensOTCentral@wales.nhs.uk.
2. Patients already known to the service
New referrals are not accepted from parents, but the service can accept referrals from parents if they have been known to the service in the past.
