Summary

The Vale Community Resource Service (VCRS), work with people in their own home to maximise functional independence in activities of daily living (ADL).  This reduces the need for admission into hospital and longer-term Social Services care services.  The service also offers therapeutic intervention and reablement support to people following an admission into hospital, so that the return home can be as timely as possible.  They provide patient-centred therapeutic intervention and support.  They work in partnership with the individual towards achieving goals that have been jointly identified.  They work collaboratively between the Cardiff and Vale University Health Board and the Vale of Glamorgan Local Authority to provide the care element of the service. 

How to access the service:

1. Referral by Health Professionals

A referral to the service is via Contact One Vale, usually completed by the Hospital or GP.  Contact One Vale can be contacted on 01446 700111 to discuss advice on accessing the service.
