    Summary

    Occupational Therapists and Physiotherapists work as part of the Community Therapy and Neuro Rehabilitation teams. The teams deliver rehabilitation on community hospital wards and in the community. The main focus of the service is preventing avoidable admissions, falls prevention and providing a Home First service by supporting people home. The team also work as part of the Virtual Wards and Reablement service.

    1.  Self Referral via Telephone / Email / Letter

    The service operates an open referral system.  You may refer yourself to the service via telephone, email or by letter.  Letters can be posted to the Occupational Therapy department at each Community Hospital site.
