Summary
The CRHT service has two main functions; they act as a gateway to the inpatient services for those assessed as a high risk or to provide intensive treatment at home as an alternative to admission. They also have a role in facilitating early discharge as evidence suggests that prolonged hospital stays have a detrimental affect on recovery.
How to access the service: You will need to be referred into the mental health teams to access the service directly. If not known to the service, please contact your GP or 111 if you need to access the Crisis Teams. If known to the service, there should be information in your care plan on how to access the service.
Out Of Hours Access Arrangements: People known to mental health services should have a care plan covering what to do in a crisis which can involve contacting the Crisis Resolution Home Treatment Team. People not known should contact their GP or GPOOH's in the first instance.
Out of Hours Telephone Number for Patients: If not a 999 emergency, contact 111 or, if known to the service, please see your care plan for details
