All referrals received by the District Nursing Service are assessed in order to confirm and prioritise the need for District Nursing intervention and where necessary signpost people to other services they may need. The District Nursing Service provides care for patients of any age. Where appropriate this is linked with specialist support when caring for patients with specialised needs.
