    Summary

    Through collaborative working and dissemination of specialist knowledge and skills, the CCN will provide support, education and care in the community. The CCN provides a link between primary, secondary care and tertiary care working to reduce admission, facilitate early discharge and minimise the need for ward attendance.

    They cover the three counties of Ceredigion, Pembrokeshire & Carmarthenshire; a large geographical area of some urban but mainly rural communities. The CCNS provides care to children and their families throughout the three counties.
