Summary
Hospice of the Valleys is in Blaenau Gwent, caring for over 600 patients every year with a team of specialist Palliative Care professionals dedicated to providing Hospice care, taking care of people’s physical needs and providing for their psychological, spiritual and social needs; helping them to live as actively as possible after diagnosis to the end of their lives, however long that may be.
The Hospice staff, includes a range of multi-disciplinary specialist Palliative Care professionals, supported by volunteers. A Consultant in Palliative Medicine, sees patients (known to the Hospice) at home, in the Day Centre or at Ysbyty Aneurin Bevan. There is a Nurse Led outpatient clinic where patients can be seen at the Hospice as outpatients by a Senior Clinical Nurse Specialist (CNS) for assessment and advice. Patients can also meet and socialise at the Day Centre. Support is provided in the patient’s normal place of residence e.g., nursing home, care home, local community hospitals (Ysbyty Aneurin Bevan and Ysbyty Tri Chwm) or via Day Centre.
The Hospice Consultant and/or Clinical Nurse Specialists can admit patients into the Community Hospital where their care is directly managed by Hospice staff working with ward staff, meaning that patients in Blaenau Gwent have access to local inpatient specialist Palliative Care. Working with the Alzheimer’s Society, Hospice of the Valleys Clinical Nurse Specialists and Dementia Support Workers support palliative and end of life care for people with dementia.
